PROJECT DESCRIPTION
Acibadem Healthcare Group is one of the leaders of private healthcare sector in Turkey and a client of IFC since 2004. Acibadem has applied to IFC for a credit worth US$ 40 million. The purpose of this project is to broaden the healthcare services by establishing hospitals in large cities of Turkey including Istanbul and Izmir. The proposed total investment amount for the period between 2006 and 2008 is estimated at US$ 100 million. The scope of this project concerns the acquisition of present hospitals and establishment of new hospitals and out-patient clinics. Acibadem who maintains a cooperation with Harvard Medical International operates 4 hospitals, 4 out-patient clinics, an eye center and a central laboratory.
ENVIRONMENTAL CATEGORY B DISCLOSURE REQUIREMENTS:
IFC requires that this document is made available through the World Bank InfoShop and to the locally affected community no less than 30 days prior to project consideration by the IFC Board of Directors. The Summary of Project Information (SPI) also provides details of where the ERS has been made available to the locally affected community. The SPI must be sent to World Bank InfoShop no less than 30 days prior to project consideration by the IFC Board of Directors.
ENVIRONMENTAL AND SOCIAL ISSUES
This is a Category B project according to IFC ''Procedures for Environmental and Social Review of Projects''. The reason behind this is that a limited number of specific environmental and social impacts can be avoided or mitigated by adhering to generally recognized performance standards, guidelines or design criteria. During the review of this project, required technical, environmental health and safety assessments had been successfully completed. In addition to these, the social information provided by the project sponsor had been evaluated and a trip to Acibadem Group's new hospital in Bursa was organized. The following potential environment, health and safety (EHS) impacts of the project were analyzed:
1. The development and application of institutionalized EHS Management system
2.The compliance of Turkish, IFC and EHS regulations by Acibadem's present and new facilities.
3.The undertaking of Acibadem for providing EHS performance in the subjects of quality, productivity and the international certification.
4.The undertaking of Acibadem for being a responsible institutional citizen
Proposed Mitigation for Environmental and Social Impacts
The sponsor of the project; Acibadem has presented plans for environmental and social impacts. The purpose of this presentation is to give guarantee for the compliance of the EHS regulations by both Turkey and IFC together with the implementation of mutually agreed specific measures. The effects of the proposed impacts on project and sponsor are summarized below:
1. The development and Application of EHS management system
Acibadem will nominate a high level EHS official within the shortest possible time.
Acibadem,at the same time will have to develop an institutional Safety and Major Fire Plan (SMFP)and implement this plan in each of all Acibadem locations.
Acibadem will have to develop an institutional Medical Waste Management System (MWMS) and implemented it in each of Acibadem locations.
Finally, Acibadem will have to develop an institutional Occupational Health and Safety Management System (OHSMS) and implement it in each of Acibadem locations.
Acibadem will satisfy the below listed requirements before the realization of first payment:
(1) Nomination of a high level EHS official for immediate duty (2) Development and implementation of its own institutional Environmental Safety Management (ESM) Major Plan endorsed by IFC (3) Development and Implementation of its own institutional (MWDMS) endorsed by IFC (4) Development and implementation of its own (OHMS) endorsed by IFC.
2. The compliance of Turkish, IFC and EHS regulations by Acibadem's present and new facilities
The wastewater of each hospital will be treated prior to being discharged to the municipality sewage system. The method and size of the prior treatment will be determined by the municipality treatment procedures. Acibadem, to the best of its ability will ensure that the municipality wastewater system works efficiently and the system provides the compliance of last processed wastewater with the Turkish and IFC guidelines.
Each hospital will use natural gas wherever it is available. In places where natural gas is unavailable an agreement will be reached with IFC for the usage of alternative energy sources.
All air conditioning systems will be compatible with IFC and Turkish regulations.
All hospital floors and their contagious and toxic wastes has to be processed and disposed according to (MWMS) endorsed by IFC. Solid, contagious and dangerous wastes will be disposed in municipality or other institutions licensed by TR. Acibadem, to the best of its ability will ensure that these institutions operate in compliance with Turkish and IFC regulations.
All Acibadem centers will be in compliance with the Safety and Major Fire Plan regulations approved by IFC.
3. Responsibility in Quality and Efficiency
Acibadem Group has developed policies and procedures which embrace all the requirements of hospital management in compliance with international standards. Since October 2003 Acibadem maintains a close cooperation with Harvard Medical School and Harvard Medical International which has been supported by 17 educational hospitals and institutions. The scope of this cooperation is mentioned below:
- Maximizing the professional development of medical personnel especially nurses.
- Development and strengthening of quality management in healthcare services
- Development of healthcare and treatment service protocols in the field of oncology and neurology.
- Application of new and advanced treatment methods and services implemented in Harvard and related hospitals.
Acibadem's medical services are managed according to the standards of USA oriented JCI accreditation. These standards are specially designed for the healthcare sector. In terms of organization, Acibadem applies EFQM standards and strives for the EFQM's Excellency Model. Acibadem has received the ISO 9001 quality certificate. It has specific regulations for patient rights. Acibadem personnel has been receiving intense and continuing training.(For example the development of technical skills, professional training on the job training etc.)
4. Social Responsibility
Private sector has been developing at a rising level in the Turkish healthcare sector. The recent changes in law indicate that more Turkish citizens will receive higher quality healthcare services from the private sector. This tendency will probably continue in future. Acibadem has been giving support to many public oriented programs. Among these, we can mention the Acibadem Education and Healthcare Foundation. This foundation concentrates on public healthcare and supports the students in medical field. Acibadem pays serious attention to child care and provides free of charge healthcare services to the children in need.
CONCLUSION
In conclusion, IFC realizes the applicability of the proposed project to Turkish and IFC environmental and social policies. At the same time, it also recognizes applicability of the environmental, health and safety guidelines upon successful implementation of the agreed mitigation measures.
MONITORING and COMPLIANCE
IFC will evaluate the project’s compliance with the applicable environmental and social requirements during the lifetime of the project by reviewing the annual monitoring reports. Annual monitoring reports will be prepared for reviewing the on ongoing performance of the project. In these reports, the compatibility of the project to the specific environmental, health and safety and social activities will be reflected annually. Periodic site supervision visits of the facilities under the scope of the project will also be included in the monitoring reports.

This Environmental Review Report includes all available environmental and social documents prepared for the project.
